Application Assistance Tools and Guidance
  • Budget Template: Use this Excel form to provide the itemized budget of a proposed project, including estimated labor and material costs to complete the project. The budget template includes Excel formulas to automatically calculate the total budget costs. Further instructions can be found in the Attach this form to your grant application.  
  • Natural Resources Screening/Project Locator Map Instructions: Refer to these instructions to create a project location map file with natural resources screening on the ANR Natural Resources Atlas.  Project locator maps indicate a projects latitude/longitude and help to identify potential natural resource conflicts. Applicants should make every effort to avoid these conflicts. If conflicts are encountered, use the Water Quality Projects Screening Tool below to contact the appropriate program.  Attach the Project Locator Map to your Dam Removal Grant Program Application. 
  • Water Quality Projects Screening Tool: This tool was created by DEC staff to screen natural resource conflicts for water quality projects in the Watershed Projects Database. The tool provides the contact information of your local Basin Planner, River Scientist, and natural resource regulatory staff including the Stormwater Analyst and the Act 250 coordinator in your area.
  • Funding Opportunities Tool: Accessible through the Clean Water Portal, this tool provides information on anticipated clean water funding opportunities, across state agencies, in the upcoming year. Program and funding information will be updated on an annual basis and as funding information changes, contingent upon agencies’ ability to report changes. Please reach out to the project contact for more information.
  • Stormwater Treatment Practice (STP) Calculator(New June 5, 2018!) This online tool allows the user to calculate estimated annual average phosphorus load and reductions associated with a stormwater treatment practice. Please refer to the STP Calculator Instructions for the limitations of this tool.
  • Clean Water Project Signs Guidance: The State of Vermont Legislature directed Vermont state agencies to post signs that identify clean water projects funded by the State of Vermont (Act 84 of 2017, Section 35a).

  • Final Reporting Grant Agreement Provisions (Attachment D in grant agreements): Explains the Clean Water Initiative Program’s grant and contract requirements, including standard milestones and deliverables and required reporting templates. (updated September 17, 2021)
  • Final Performance Report Template (Attachment E in grant agreements): The individual project level performance reporting template (updated October 16, 2019).
  • Block Grant Final Reporting Template: This is a required final deliverable for all Clean Water Initiative Program block grants. This Excel file replaces the Final Performance Report for block grants and includes the Stormwater Best Management Practice Report and the Riparian Buffer Planting BMP Report. Please enter each design and implementation project with a WPD ID as its own row in this file. All projects should already have been entered to the Watershed Projects Database via the Batch Import File (update November 9, 2020).
  • Batch Import File (BIF): Please "enable content" before using this file. The BIF is used by partners to upload multiple clean water projects into the Watershed Project Database. It is a required deliverable for block grants and some assessment project types (e.g. Stormwater Master Plans). Please contact your regional watershed planner or your technical project manager before submitting the BIF so they can help ensure the accuracy of project information, avoid entering duplicate projects and answer questions (updated February 9, 2021).
  • Unified Scoring Prioritization for Stormwater Master Plans (SWMPs): This document provides a standardized method to score and prioritize projects identified in SWMPs.  It is to be used in all SWMPs going forward. (added 9/11/2020)
  • Outreach Tracking Instructions and link to reporting form: This form is a requirement for all outreach activities conducted within the scope of an Ecosystem Restoration Grant.
  • Operations & Maintenance Plan and Agreement Template: This is a required form for implementation projects requiring O&M plans and agreements for grants utilizing Clean Water Initiative Program funds (updated December 11, 2019).
  • Third Party Operations & Maintenance Plan and Agreement Template: This is an alternative O&M Agreement Template for use with projects where the O&M responsible party (e.g., watershed group) enters into a separate agreement with the project landowner, and the O&M responsible party agrees to act as an intermediary between DEC and the landowner. To use this form the following must be true: The Responsible Party agrees to enter into a separate O&M agreement with the Landowner and to keep the signed agreement on file for the duration of the O&M agreement (template updated February 28, 2020).
  • Stormwater Best Management Practice (BMP) reporting template: This form is a required deliverable for all ERP funded projects that completed the final design or implementation of a Structural Best Management Practice (BMP). This report pertains to mainly stormwater BMPs. Complete a separate report for each BMP completed within the scope of your project and submit along with the Final Performance Report. The following guidance document provides BMP definitions and storage volume calculations for each practice type BMP Storage Capacity Calculations (updated April 2, 2018).
  • Riparian Buffer Planting Best Management Practice (BMP) reporting template: This form is a required deliverable for all Clean Water Initiative Program grants that installed buffer planting(s) within the scope of their grant (updated September 17, 2021).
  • Transfer of Ownership Request for Equipment Template:  Any equipment purchased or furnished to a grantee or sub-grantee by the State under a grant agreement is provided on a loan basis only and remains the property of the State. The grantee must submit a written request to retain the equipment at the end of grant term for the same use and intended purpose as outlined in the applicable grant agreement. The written request should include: description of equipment, date of purchase, original cost and estimated current market value. Complete the template by replacing the wording in brackets [ ] and signing the letter. (uploaded 11/25/2020)
  • Municipal Roads Grants-in-Aid Final Performance Report (best management practices): This form is a required deliverable for projects involving installation of road erosion control best management practices (updated June 22, 2018).
  • Municipal Roads Grants-in-Aid Final Performance Report (equipment): This form is a required deliverable for projects involving purchase of equipment that supports installation of road erosion control best management practices (updated June 22, 2018).
  • Vendor Guidance for ANR Online - Grants & Contracts: This link is directed to a webpage with resources to assist vendors who have a grant or contract with DEC on how to properly use ANR Online for agreement payment (updated July 30, 2020).
  • Form 430-M: This form is a required deliverable for all grants that committed "match" or leverage funding as a part of the agreement.  
  • Project Closeout Checklist: This form is for use by Technical Project Managers (TPM's) at the closing of a grant to ensure all the required information was gathered to track and report on grant outcomes (updated November 29, 2018).
