Contamination found during a tank removal/closure is to be reported to the Waste Management and Preventions Spill Program by calling 802-828-1138.

This form along with written summary of the findings of the tank closure are to be emailed to susan.thayer@vermont.gov or upload to the FTP site (instructions for uploading documents) and place in the file folder titled "Storage Tank and Components Closure Reports"(please notify Susan Thayer that a report is in that folder)
